Notice Regarding Affordable Care Act (ACA) Reporting: Availability of Form 1095-C

In accordance with the Paperwork Burden Reduction Act, please be advised that your annual Affordable Care Act (ACA) health coverage statement (Form 1095-C) is available. Although this form is not automatically furnished, you may obtain a copy of your specific statement upon request.

Current employees may access their Form 1095-C on the Tyler IV employee portal under the current year's tax documents.
